COMS Interactive and Aperion Care to Provide Evidence-Based Care Content Together to Skilled Nursing

The clinical care and quality improvement leader in the senior care market and the post-acute organization based in Illinois have joined hands to bring evidence-based care content to skilled nursing.

-- Aperion Care operates 41 locations across three states, including 33 Skilled Nursing Facilities that will implement the COMS technology.

The post-acute organization based in Illinois has seen a sharp increase in the acuity of its residents. The leadership of Aperion Care made the proactive decision to use technology to enhance clinical care and reduce unnecessary hospital readmissions. The general need for clinical transparency reporting that directly impacts a senior care organization’s bottom line has grown with value-based healthcare and the rise in resident acuity.

The COMS and Aperion Care partnership will leverage technology to achieve these outcomes and position Aperion Care as a leader in post-acute care. Aperion Care has begun the implementation process in 33 Skilled Nursing Facilities.
