Due to COVID-19, Expert Home Care of New Jersey has launched a new program focusing on transitioning seniors residing in assisted living facilities and nursing homes to NJ live-in home health care.

Frank Esposito, Executive Vice President of Expert Home Care, Inc. said the program includes a professional transition team to handle many of the details involved with moving a senior into a new care environment, in this case, a senior’s or relative’s own home.


According to Esposito, the NJ Live-in Home Care Transition Program includes assigning a case manager to oversee all of the tasks related to moving a senior loved one from an assisted living facility or nursing home to a live-in home care arrangement.


“The team includes a Registered Nurse who sets up a care plan and oversees the transition, ensuring continuity of care” said Esposito.
