Maryland Self-Directed Home Care Plans For Developmental Disability Updated

Community Health and Education Services (CHES) (240-755-5591) has updated its self-directed in-home nursing support services for Maryland residents with developmental disabilities.

Included in the organization's recently updated self-directed care services are nursing consultation and nursing case management and delegation. Depending on a patient's Health Risk Screening Tool (HRST) level, medication and care needs, current caregiver situation, and other factors, CHES can offer one of these types of self-directed care services and tailor it to meet the patient's unique condition.

CHES' nursing consultation services are administered by a Registered Nurse (RN) who has completed the DDA-approved RN training. This type of self-directed care includes a comprehensive nursing assessment, a self-administration assessment, and an HRST rating or reviewing session with the patient, nurse, and involved family members. Individuals receiving nursing consultation get assistance in developing their healthcare protocols and routines, as well as support in identifying community healthcare resources.

The nursing case management and delegation service is required if a patient's paid staff or family members will be administering medication or completing healthcare treatments. With this type of self-directed care, the dedicated DDA-approved RN develops the patient's care plan with input from the patient and their family or staff. The RN also trains a patient's staff to complete all the tasks listed on their care plan, then delegates to and supervises the staff.

The nursing consultation service from CHES includes up to four visits, with a maximum of 16 hours, per year, while the nursing case management and delegation service includes nurse visits at least every 45 days. Individuals in Maryland who have cerebral palsy, autism, spina bifida, or other developmental disabilities can visit the CHES website to determine which type of self-directed nursing service is best for them.
