Lanham Self-Direction Support Broker Services For Choice & Flexibility Announced

Community Health and Education Services’ (CHES’s) support broker services facilitate communication and coordination between patients, their families, and their service providers such as personal care assistants (PCAs), registered nurses, and DDA representatives.

The agency’s support broker services focus on individuals who have opted into a self-direction program. CHES staff will work with these individuals and their primary care providers to create customized care plans that best meet each patient’s needs and goals. Agency professionals are trained to assist clients with cerebral palsy, spina bifida, and those on the autism spectrum.

In addition to creating care plans, support brokers can assist self-directed patients with managing their budgets and with navigating and accessing available resources. Through CHES, these resources include skilled nursing, personal care, and Developmental Disability Agency (DDA) nursing support services.

With CHES, patients who participate in the DDA’s self-direction program can utilize nursing consultations or nursing case management and delegation services. The latter services are required for patients with family members who are considered paid staff and who administer medications or treatments.

CHES is headed by Dr. Leslie Phillips and Lisa Noel, MSN, RN, NP-C, Director of Operations. Dr. Phillips brings 30 years of clinical experience as a registered nurse, and Ms. Noel brings 26 years of experience working in pediatric and adult healthcare. Dr. Phillips has developed a pioneering comprehensive functional status assessment tool for clients, and Ms. Noel is the co-founder of a clinic program to improve community members’ health and quality of life.
