CHES services are designed to cater to both children and adults with developmental disabilities in Montgomery County, Prince George’s County, and surrounding areas. Operating under Medicaid's Community First Choice initiative, CHES enables Medicaid participants to access essential home-based care services.
Individuals with IDD who are eligible for self-directed care based on their diagnosis and psychological reports can now choose and manage their home care providers either directly or through their authorized representative.
Skilled nursing is a key component of CHES's updated in-home care services. Registered Nurses oversee or serve as visiting nurses, managing cases and providing a range of medical services, including nutrition monitoring, vitals assessments, wound care, post-surgical care, diabetic care and education, and pain and symptom management to improve clients' comfort levels.
Another important aspect of CHES's new at-home services is personal care. Certified personal care assistants assist clients with daily tasks such as bathing, eating, light housework, and mobility exercises. They also provide support with meal planning, grocery shopping, medication reminders, and status reporting to clients' families.
CHES emphasizes the importance of personalized care plans tailored to meet the unique needs of each individual. By working closely with service users and their primary care providers, CHES formulates customized care plans for IDD adults and children.
